Structure in Fives

A Québec teaching-hospital chart separates physicians by specialty, yet one operation joins a surgeon, anesthesiologist, nurse, facilities, and patient. Henry Mintzberg builds a language for that hidden coordination, then proposes five ideal configurations; specified fit-effectiveness hypotheses did not receive support in a 1993 empirical test.

The patient crosses the boxes

One of the charts in Henry Mintzberg's Structure in Fives depicts the medical component of a Québec teaching hospital. Physicians are grouped by specialized knowledge: surgeons in one branch, anesthesiologists in another, and other specialties in their own units. The boxes make formal grouping legible. They do not, by themselves, show how an operation happens.1

For that, the book asks the reader to follow the work. An operation joins at least a surgeon, an anesthesiologist, and a nurse. The room and instruments must be ready; information must travel across specialties; someone must respond when the patient's condition changes. Mintzberg adds an illustrative kidney patient who develops heart trouble: the need does not wait politely inside the unit that first received it. This is not a reported clinical outcome. It is an authored example that reveals the organizational problem the chart conceals.2

The hospital coordinates much of this work before the participants meet. Professional education standardizes skills and classifications; shared clinical knowledge lets one specialty anticipate another. Yet training cannot specify every event. In the operating room, people also adjust to one another in real time. Hospital management can allocate rooms and set rules, but direct supervision is a poor substitute for expertise when the work becomes complex. Structure, in Mintzberg's account, is the whole settlement by which divided work is joined—not merely the drawing of who reports to whom.3

A pottery fable compresses an organization's transformations

Mintzberg opens the book with Ms. Raku, an invented potter whose studio grows into Ceramico. At first a few people coordinate informally. Growth brings mistakes, including a kiln of hanging planters glazed the wrong color, and Raku appoints a studio manager to supervise. A work-study analyst later divides wedging, forming, tooling, glazing, and firing into standardized jobs. As the fictional company expands into tiles, bathroom fixtures, and bricks, divisions and quarterly performance targets replace the founder's direct reach.4

The fable is memorable because every transition solves one problem and creates another. Informal conversation preserves discretion but becomes unreliable at scale. Supervision restores a focal point but overloads the supervisor. Standardized processes create repeatability while narrowing each worker's view of the object being made. Divisional targets let a distant center compare businesses while filtering local reality into numbers. The story illustrates the theory; it is not evidence that real firms inevitably pass through these stages. That reading is an interpretation of the sequence, not a result measured by the fable.5

From the pottery and hospital examples, Mintzberg derives five coordinating mechanisms. Mutual adjustment lets people adapt through communication. Direct supervision places responsibility in one person. Standardizing work specifies the process; standardizing outputs specifies the result; standardizing skills relies on prior training. Several can coexist, but a different one tends to carry the hardest coordination problem in a given setting.6

Parts become powerful through the mechanism that dominates

The five parts in the title are positions in an organizational argument. The operating core performs the basic work. The strategic apex carries overall responsibility, joined to operations through the managerial middle line. The technostructure creates standards, plans, and controls without necessarily supervising the people who follow them. Support staff provide services outside the basic operating flow.7

Each part gains influence through a favored kind of coordination. A founder at the apex can pull toward direct supervision. Analysts in the technostructure gain reach when processes are standardized. Skilled operators defend autonomy when professional training is the basis of coordination. Middle managers gain room when outputs are assigned to divisions. Specialists collaborating on novel work depend on mutual adjustment. A redesign therefore changes more than an information flow: in this interpretation, it changes whose knowledge counts and who may interrupt whom.8

Mintzberg clusters these tendencies into ideal configurations. A young organization under a strong leader may resemble a simple structure. Stable, repetitive mass work can pull toward machine bureaucracy. A hospital or university can resemble a professional bureaucracy because trained operators control much of the work. A large diversified company can become a divisionalized form, with headquarters governing units through output targets. Novel, interdependent work can call forth adhocracy, in which specialists coordinate laterally. These are coherent pressures, not five bins into which every organization must fit.9

The hospital is democratic for some people and bureaucratic for others

The teaching-hospital episode also reveals the framework's politics. Mintzberg describes professional bureaucracy as decentralized because physicians and other credentialed operators retain substantial authority over their work. They can influence hiring, standards, resource allocation, and administration through professional bodies and committees. The same organization may give support workers a parallel, top-down hierarchy governed by rules. What looks democratic from the physician's operating core can look like machine bureaucracy from a porter's, cleaner's, clerk's, or technician's station.10

Even professional autonomy is conditional. Training institutions and associations define acceptable practice before a clinician enters the hospital. Mintzberg reports a McGill research example in which a medical association treated failure to adopt a new method as tantamount to malpractice. Expert discretion is therefore produced and constrained by institutions outside the chart.11

Patients and families are not a sixth organizational part in the model, and the hospital discussion does not document their authority over structural design. That source boundary matters: the patient is used to explain professional coordination, while the patient's own account of handoffs, delay, error, or appeal is not investigated.12

The framework therefore supports questions rather than measured impact claims. When does standardization carry safety knowledge, and when does it suppress situated judgment? When does professional autonomy protect care, and when does it shield a guild from scrutiny? When do divisional targets clarify responsibility, and when do they reward a unit while the patient bears a cost at the seam? Structural fit asks whether elements reinforce one another; it does not, without an additional ethical judgment, establish that the purpose or distribution of power is good.13

The five forms are a synthesis, not a confirmed natural law

The 1983 book is a shorter version of Mintzberg's 1979 The Structuring of Organizations and synthesizes a broad literature on organization design. His 1980 Management Science article presents the configurations as a synthesis of parts, mechanisms, design parameters, and contingencies. The organigrams, studies, and industry examples inform that synthesis, but the five ideal forms were not discovered by classifying a single representative sample of organizations.14

Mintzberg closes by conceding that real organizations are more complex than the theory and that the descriptions deliberately overstate their tendencies. A pharmaceutical company, he later noted, can be adhocratic in research, professional in development, and machine-like in production. The seams matter as much as the dominant form.15

A later test by D. Harold Doty, William H. Glick, and George P. Huber translated the five forms into quantified profiles and used top executives' reports from 128 U.S. organizations; 85 organizations responded again one year later. The cross-sectional correlations between closeness to an ideal type and five effectiveness measures were not significant. One of five lagged correlations was small but significant, and the authors still judged the longitudinal hypothesis unsupported.16

The test does not settle every use of the framework. It relied on one top-manager informant per organization, quantified the ideal profiles through two experts, and did not measure every component of Mintzberg's theory. It also tested the configurations as organizational profiles, not every use of them as simultaneous forces or as diagnostic vocabulary.17

The defensible conclusion is narrower than either celebration or dismissal. Doty and colleagues provide a serious negative test of the specified fit–effectiveness claims; its operationalization leaves other interpretations open. The configurations remain a diagnostic language, while a general claim that resemblance to the five ideal types produces effectiveness lacks support from that test.18

Relations and reading paths

Henry Mintzberg is the work's creator. The Nature of Managerial Work, his 1973 study of managerial activity, is an author-progression prerequisite: observation of managerial work comes before the later structural taxonomy. The shared author and chronology are documented; the reading order is editorial.19

Strategy and Structure is a historical-to-structural editorial comparison: Chandler follows the history of divisionalization, while Mintzberg places the divisionalized form in a wider typology. Culture, informal organization, trust, and voice is an analytical extension for examining coordination that exceeds formal grouping. These routes do not assert that Mintzberg adopted either linked framework.20

Organizational intelligence is an analytical lens for asking how a structure senses and coordinates. Benefit for all life is an ethical audit lens for asking who benefits, who bears burdens, and which effects fall outside the five internal parts. Both are editorial relations, not concepts named or tested in the cited work.21
